Request Forms

Account Request Forms:

     MUINFO Account Request Form and Information
     Information and forms to request MUINFO accounts and User Profile
     requests for specific Banner forms and tasks.
     NOTE: The completion of Banner Basic Navigation Training is
     required prior to the issuance of an MUINFO account.

     Account Termination/Departmental Transfer Request Form
     Form to terminate an employee's account if an employee has
     resigned or has transferred to another University department. You may
     complete form on-line, print, sign, and mail to Computing Services.

General Person Entry Completion Form
Form to request privileges to enter General Person records in Banner.

Report Request Forms:

     Adhoc Report Request
     Request form to have existing reports produced, modifications made
     to existing reports, or new reports written.

Banner2000 Software Modification Request Forms:

Baseline/SCT Banner Software Modification Request
Request for modification to the baseline Banner2000 product.

     Request for development of new MU Banner Software
     Request for software to be developed by Marshall University
     in support of Banner2000

MU Banner Software Modification Request

Request for modifications to software written by Marshall University
in support of Banner2000.

Change Assessment/Request Form
Request form for any Network, Hardware, or Software changes that may affect the Banner INB and SSB applications. Complete the form and submit to the BOC for review.

Banner Account Request Forms

For authorization to access Banner, you must complete the MUINFO Account Request Form and the appropriate Banner User Profile Form(s) below. The User Profile Form identifies those forms and processes in Banner for which you need access to perform your job functions. You only need to complete the MUINFO Account Request form once. You may, however, submit multiple Banner User Profile Forms at anytime once you have completed the MUINFO Account Request Form.

About the MUINFO Account Request:

Account Request Form is required for first-time BANNER users

Basic Navigation Training must be completed prior to issuance of account; register for a Basic Navigation Course

Grants login access to the BANNER application and the MU Network

Requires you to read and accept the Marshall University Acceptable Use and Policy

Requires your Supervisor, Dean, Department Head, or Vice-President's signature

Requires the signature of your Basic Navigation training instructor

Requires your Dean, Department Head, or Vice-President's signature

Banner Financial Information System User Profile Form

Finance Information Systems privileges
Electronic Approvals privileges
For Requisition and Encumbrance Document entry
For inquiry on budget and accounting information

Banner Human Resource Information System User Profile Form

Banner Student Information System User Profile Form (not yet developed)

Student Information System privileges
Have your supervisor E-mail Terri Tomblin-Byrd, Computing Services, tomblin@marshall.edu requesting access to Banner forms you need and the type of access (query/view only or update).
