EPAF CATEGORY INFORMATION SHEET

CATEGORY: APT015 
PURPOSE: Used to appoint a NEW Work Study with Termination Date

NECESSARY CONDITION:
It is necessary to submit the following Category, TRM010,  immediately following the submission of Category APT015.

USAGE:
This Electronic Personnel Action Form (EPAF) Category, APT015, is used to appoint a NEW Work Study with a Termination Date.  The second Electronic Personnel Action Form (EPAF) Category, TRM010, is used to terminate the employee record of the Work Study.  NOTE: It is not necessary that the employee never have worked for Marshall University at any time in the past.  It is only necessary that - at the effective date of the EPAF - the individual does not have any other current job in Banner HR.  

APPROVERS:
This EPAF Categories requires review and approval by the Dean/Director (DNDIR), Financial Aid Approval (FINAID), Vice President (VPRES), the Budget Office (BUDGET), HR Quality Assurance (HRQA), Payroll Office Review (PAYREV - SUE FERRELL), and the Payroll Office (PAYROL).  Note that the Dean/Director is not necessarily whom the EPAF originator reports but are rather the person responsible for the position that is the subject of the EPAF.  The latter reviewers/approvers are attached to the University administration and act in behalf of all EPAFs regardless of originating department.  Note also that the originator of the EPAF and the Dean/Director and Vice President approvers (1) must be enrolled for Banner HR views of employees, positions, and jobs; (2) must be enrolled to participate in the EPAF process; and (3) and must be enrolled as necessary for their respective approver role in the EPAF process.  The originator of the EPAF is not an approver role and requires no application - just access to the EPAF process.

BACKGROUND INFORMATION REQUIRED:  Originators of this EPAF Category are asked to gather the information below before attempting to create and send this EPAF.  This web page can be printed out if it would be helpful as a place to record the necessary information.
