Application for Admission to CD 570/571, 670/671, 672/673
Due to the increasing number of students requiring clinical practicum assignments, and in an effort to continue to provide appropriate clinical practicum sites, graduate students will be required to apply for admission to CD 570/571, 670/671, 672/673. Applications, which must be approved by the student's advisor, must be submitted to the Clinic Director in accordance with the following schedule:

Admission to Fall Semester:

Application due July 1st (Application may be withdrawn or amended without penalty if a written request is received by the Clinic Director no later than 4:30 p.m. on August 10th.).
Admission to Spring Semester:
Application due November 1st (Application may be withdrawn or amended without penalty if a written request is received by the Clinic Director no later than 4:30 p.m. on January 2nd.)
Admission to Summer Semester:
Application due April 1st (Application may be withdrawn or amended without penalty if a written request is received by the Clinic Director no later than 4:30 p.m. on May 15th.)

Students planning to submit an application must see their advisor for scheduling prior to the application deadline.

Applications received after the deadline for submission will be given consideration for admission to CD 570/571, 670/671, 672/673; however, applicants may not be assured that this request will be granted. Student who withdraw an application following the specified grace period and students who fail to register for the approved amount of practicum will not be given priority consideration for admission to CD 570/571, 670/671, 672/673 in subsequent semesters.

Should students need to make changes in their original application after the specified grace period has lapsed, a written request, including reasons for the request, must be submitted to the Clinic Director. These requests will be given due consideration although no guarantee of approval of the request can be provided. If the request is denied and the student cannot fulfill the responsibilities of their original request, their application will be considered withdrawn and they will not be given priority consideration for admission to CD 570/571, 670/671, 672/673 in subsequent semesters.

NOTE:  Because fall student teaching (CD 672/673) placements are often made in the spring, deadlines for application to and withdrawl from CD 672/673 may differ from those stated above (see application form).
