If the proposed course is not currently in the catalog please provide a copy of the Faculty Senate approved course recommendation (for Undergraduate courses) or the Graduate Council approved course recommendation (for Graduate courses) when you submit this form. Approvals or non-approvals must be returned to the FDCOMI chair.
