APPLICATION INSTRUCTIONS
PARAMEDIC CERTIFICATION PROGRAM

Please follow all instruction on this sheet as listed. Failure to follow instructions may void or delay your entrance into the Paramedic Program.

1. The following materials must be RECEIVED at the following address:

Paramedic Science Program
Marshall Community and Technical College

Huntington, WV 25703-1527

Paramedic Program Application (enclosed)
Copies of CPR card front/back, EMT-B card and driver’s license.
Photocopy of high school and/or college transcript.
Submission to the Program Coordinator of a completed immunization record prior to participation in any Clinical Rotation.
Submission of a yearly TB test result.
Must make application to Marshall Community & Technical College and be accepted.

2. Do not submit any materials other than those listed above. Others may be required later.
