In order to receive services, students must provide documentation to the ODS office and meet with the director and/or coordinator before the semester begins.
Types of Documentation
Students with a:
- Medical Disability
- Visual Impairment
- Hearing Impairment
- Orthopedic Disability
will need a statement, on office letterhead, from a medical doctor stating the following:
- the student’s disability
- how the disability will affect the student academically.
- If at all possible this statement should include recommendations. e.g. needs to sit in the front of the class, etc.
- medications that the student may be taking
- how that medication might effect the student
- doctor’s signature & date
This statement should not be written on a prescription pad.
Students with a:
- Learning Disability (ADD, ADHD, Dyslexia, LD)
- Psychological Disorders
will need a psychological evaluation administered by a licensed psychologist which includes:
- Diagnosis
- Recommendations for classroom accommodations
- Medications student is taking (if any)
- How this disability will effect students academically
Documentation should be no more than 3 years old.
504 Plans and IEPs are not acceptable.
