Program of Study Forms

The forms for the program of study are major specific and are created as pdf documents. You can type into the document and then print it out, or you can print out the document and then write or enter the information with a typewriter.

If you type onto the document on your computer, you cannot save a copy of the file with the things you have entered unless you have Adobe Acrobat (the full version, not just Reader).

Be sure to choose the correct form...

For students admitted prior to 2006:

     Program of Study, all majors, admitted prior to 2006

For students admitted in 2006 and later:

     Curriculum and Instruction

     Educational Leadership - Higher Education Administration

     Educational Leadership - Public School Administration
