Complete the forms below to request informational corrections that will reflect in Banner, Outlook, and other systems on campus. There are three sections of data that can be updated.
- Preferred First Name – This is to request a correction to your display or preferred name. Legal Name changes will require documents to be presented to MURC.
- Work Address/Phone Number – This is to request a correction to your work (WK) address or phone number.
- Supervisor/Home Org – This is to request a correction to your supervisor/home org data in Banner. This form is not to be used to change your departmental organizational structure. It is for informational purposes only.
