Note: This form must be filled out completely in order for the reservation request to be valid.

Please make sure that you are familiar with the revised Collaboration, Team and Study Room Scheduling Procedure before reserving a room.

The Reservation Request Form will not send unless all the required fields are included and correct. Once successfully submitted, you will be redirected to a thank you page and a copy of the request will be sent to the email you provided. If these two things did not happen, then your request was not submitted successfully. You will receive a confirmation or denial of the reservation through email within 24-hours.


  • Date and Time Requested


    Rooms will be scheduled in 2 hour blocks.
    An individual may request up to three blocks (six hours) in a calendar month, with no more than two blocks (four hours) in a 24-hour period.
