Prior to scheduling an evaluation, a case history form must be completed and returned to the Clinic office. Treatment recommendations are based on the results of the evaluation and scheduled accordingly.
Marshall Health serves as the billing intermediary for the MUSHC. Often a referral from a physician is required to meet regulations from health insurance providers.
