Application for Health-Care Professional School 

A lot of the information you need to prepare your applications can be found on this Pre-Health Care Professional web site.

  • Links to the professional schools of which we are aware are listed under Professional School Links.

  • Information on the Application Submission Services is listed under Application Services.

  • A personal statement is required by some of the applications services. Information on writing the personal statement can be found under Helpful Hints.

Download all of the forms provided below. It is your responsibility to see that they are all completed and submitted to the Dean’s Office, S270, to be placed in your application file. Documents you need to submit for that file are itemized below. You can add information in Microsoft Word and print the downloaded documents. A personalized, tailored, and complete cover letter of recommendation will be written by the Associate Dean who also serves as the Chief Pre-Health Care Advisor based on your resume, the personal statement, and letters of recommendation from a minimum of three faculty members. The required forms are as follows:

When applications are complete you may ask that the materials and letters be sent to each of the schools you list on the Evaluation Request Form. Check with the Dean’s Office to be certain your file is complete. If letters of recommendation are not submitted you may have to remind your professors.

To open your application and to initiate your application file, please follow these steps:

  1. Complete a copy of the resume form by adding your information to the Resume Form template. Delete anything from the form or add anything that is appropriate for you.
  2. Attach a picture to your resume.

  3. Complete the Evaluation Request Form. Professional Schools in excess of 10 must be approved by the Associate Dean/Chief Pre-Health Care Advisor.
  4. The file containing Recommendation Forms contains three copies. Complete the top portion and give them to your references to complete. The professors serving as references will return them to the S270.
  • It is the student’s responsibility to keep up with the status of their evaluation request. Office staff will attempt to inform the student when a recommendation has been received on their behalf, via email.

  • To check the status of the evaluation request or to have a completed evaluation sent to a professional school, you should contact Teresa Holschuh at 696-2372.

  • Pre-Professional Evaluations are not automatically sent to the professional schools upon completion of the Evaluation Request. You must request that the materials be sent to specific schools.

  • If a student chooses to use letters of recommendation from a previous year, they may do so by submitting a written request (e-mail will suffice).

  • Letters of Recommendation will remain on file in the College of Science for five years from the last year they were sent to professional schools.

  • If you are applying to Allopathic Medical, Osteopathic Medical, Dental, Pharmacy, or Veterinary school you will be required to submit some credentials via an application submission service. You may find information about the services for the Application Service link.

  • If the school to which you wish to apply does not use a submission service you need to consult each institution about the procedure for application submission.

  • The CoS is not responsible for sending academic transcripts; you must contact the Office of the Registrar. You must request, in writing (not e-mail), an official transcript from the Office of the Registrar. If you are applying to a school that requires the use of an application service the transcript has to be sent to the service.

  1. Make an appointment with the Associate Dean when your application is complete. We need to confirm all information that will be included in your cover letter of recommendation
