INSTRUCTIONS

  1. Fill out the form with your data. These should be completed every Friday (Note: Do not refresh or reload the page while you are working, because it will automatically reset itself and you will lose your data.)
     
  2. When you have completed filling out the form, click the button, found on the toolbar above the spreadsheet. Clicking this button will launch Microsoft Excel.
     
  3. Excel will open. Your spreadsheet will appear.
     
  4. You will then save your spreadsheet to your computer, or floppy disk.  In Excel, click the File menu and then click Save As. Select and save the spreadsheet as a Excel Workbook file with the extension .xls. You will be prompted to specify a location to save the file. You also need to specify a file name.  Make sure the file name includes your name and the date.  Although you are completing the form on Friday, you should name the file for the Monday of that week.  For example: beard021604.
     
  5. If you are having difficulty entering your data, you may need to install an up-date to your computer.  Click on the link - Office Web Component - and follow the directions to install this up-date.
     
  6. Once you have saved the file, e-mail it to the clinic director, Dr. Keith Beard, as an attachment.  Be sure to e-mail a copy to your supervisor, as well.
