1. Request by mail by calling 1-800-438-5391 or (304) 696-6768 or by sending an e-mail to residence-services@marshall.edu
2. Pick one up in the Department of Residence Services located in Holderby Hall on Marshall's Huntington Campus
3. Download the application online:

4. Access, complete and submit electronically by clicking the link below:

2. Return completed application to the Department of Residence Services (if not submitted electronically) to:

3. A Department of Residence Services Staff member will contact you, or you can call the Residence Services office, to verify the submission of your application.
