Submissions will be accepted for academic year 1996 to present.

1. Creative Works:
   Subheadings: Articles, Books, CD's, Consulting Editor, Review Work, Web Page, etc...
   
   Authors or Artist. (Year/Date). Title or description. Location of work.
     
2. Research Based:
   Subheadings: Articles, Funded Projects, Presentations, Proceedings, Publications, etc...
   
   Authors. (Year). Title. Where it was presented with location or Journal Title, volume, and/or number with page numbers or Book Title with Publisher name.
     
3. Professional/Community Service:
   Subheadings: Memberships-private or professional, Offices Held, etc...
   
   Name. (Year/Date). Title or brief description of activity. Location of activity.
     
4. Awards:
   Subheadings: Professional, Personal, etc...
   
   Name. (Year/Date). Title of award or brief description. Who granted the award or name of organization.
     
5. Other:
   
   Name. (Year/Date). Description.
