The following are a list of resources that faculty may need quick access to:
- Facilities Use Request- Request to use forensic science resources/facilities
- Room Scheduling Calendar – Check available rooms for reservation, submit requests to Program Staff.
- Special Activities Report Form - Turn these in after each special activities so we may include them in the FEPAC and MU Annual Review.
- Request to Use Facilities - Please complete this form when a non-MU agency uses the MUFSC Facilities
- Facilities Use Waiver- Please have any underage persons sign this waiver if using the MUFSC Facilities

- MUFSC Travel Request | Travel Policy and Procedures - Please submit THREE (3) WEEKS PRIOR TO TRAVEL
- Supervisor KSA Evaluation - To be completed for your GA/WS/Intern students at the end of each term.
