The Center of Excellence is responsible for the management of the Transformational Collaborative Outcome Management (TCOM) tools in West Virginia. It is an approach grounded in a philosophy of a single shared vision–helping people achieve their health and wellness goals as they navigate healthcare, child welfare, juvenile justice, behavioral health, education, and other complex systems.

• Training and Technical Assistance – The Center provides training and technical assistance for the Child Adolescent Needs and Strengths tool (CANS) and the Family Advocacy and Support tool (FAST). These tools are designed to look at multiple points of view, and a rating is assigned to each item that addresses the level of action needed to build a clear, mutually agreeable action plan.  
• Other TCOM Activities: Latent Class Analysis, WV Decision Support Model, Continuous Quality Improvement, WV TCOM tool development and Wraparound Fidelity Reviews
