Speech and Hearing Center provides comprehensive swallowing evaluations, including <strong>Flexible Endoscopic Evaluation of Swallowing (FEES)<\/strong><strong>,<\/strong> for adults experiencing difficulty eating or drinking safely and efficiently.<\/p>\n<p>FEES allows our clinicians to directly observe swallowing using a small flexible camera. You then eat and drink selected foods and liquids while the clinician observes your swallowing.<\/p>\n<p>FEES allows the clinician to evaluate:<\/p>\n<ul>\n<li>Structures of the throat and larynx<\/li>\n<li>Management of saliva and secretions<\/li>\n<li>Movement of food and liquid through the throat<\/li>\n<li>Swallowing safety<\/li>\n<li>Penetration or aspiration<\/li>\n<li>Residue remaining after the swallow<\/li>\n<li>Effectiveness of different swallowing strategies<\/li>\n<li>Changes in swallowing across repeated trials<\/li>\n<li>Which food and liquid consistencies may be safest or most efficient<\/li>\n<\/ul>\n<p>The results help guide recommendations for diet, swallowing strategies, and treatment.<\/p>\n<p>&nbsp;<\/p>\n<h2>What to Expect During a FEES Evaluation<\/h2>\n<p>Before the procedure begins, your clinician will review your medical history, current swallowing concerns, medications, diet, and any previous swallowing evaluations.<\/p>\n<p>During the procedure:<\/p>\n<ul>\n<li>A small flexible scope is passed through one side of the nose<\/li>\n<li>The scope remains above the vocal folds and does not enter the airway<\/li>\n<li>You will be asked to eat and drink different foods and liquids<\/li>\n<li>Your clinician will observe how safely and efficiently you swallow<\/li>\n<li>Different strategies or positions may be tried to determine whether they improve swallowing<\/li>\n<\/ul>\n<p>The procedure does <strong>not require radiation<\/strong>, so swallowing can be observed repeatedly as needed.<\/p>\n<p>After the examination, your clinician will review the findings with you and discuss recommendations.<\/p>\n<p>&nbsp;<\/p>\n<h2>What Can We Learn From FEES?<\/h2>\n<p>A FEES evaluation can provide detailed information about swallowing function and help answer questions such as:<\/p>\n<ul>\n<li>Is food or liquid entering the airway?<\/li>\n<li>Is material remaining in the throat after swallowing?<\/li>\n<li>Are certain food textures or liquid consistencies easier or safer to swallow?<\/li>\n<li>Does changing head position improve swallowing?<\/li>\n<li>Do swallowing strategies reduce aspiration or residue?<\/li>\n<li>Is fatigue affecting swallowing during a meal?<\/li>\n<li>Is the person managing saliva and secretions effectively?<\/li>\n<li>Would swallowing therapy be beneficial?<\/li>\n<\/ul>\n<p>Recommendations are individualized based on both the examination findings and the person&#8217;s medical condition, preferences, and everyday needs.<\/p>\n<p>&nbsp;<\/p>\n<h2>FEES vs. Modified Barium Swallow Study<\/h2>\n<p>Both <strong>FEES<\/strong> and the <strong>Modified Barium Swallow Study (MBSS)<\/strong> are instrumental examinations used to evaluate swallowing.<\/p>\n<h3>FEES<\/h3>\n<p>FEES:<\/p>\n<ul>\n<li>Uses a small flexible camera passed through the nose<\/li>\n<li>Does not use radiation<\/li>\n<li>Allows direct visualization of the throat and larynx<\/li>\n<li>Allows extended or repeated trials when clinically appropriate<\/li>\n<li>Can be particularly useful for evaluating secretions and fatigue<\/li>\n<li>Can be completed in the clinic and, in some circumstances, at other care facilities<\/li>\n<\/ul>\n<h3>Modified Barium Swallow Study<\/h3>\n<p>An MBSS:<\/p>\n<ul>\n<li>Is completed using X-ray<\/li>\n<li>Requires swallowing foods and liquids mixed with barium<\/li>\n<li>Allows visualization of the mouth, throat, and upper portion of the esophagus during swallowing<\/li>\n<li>Provides a different view of swallowing physiology than FEES<\/li>\n<\/ul>\n<p>Neither test is universally &#8220;better.&#8221; The appropriate examination depends on the individual&#8217;s symptoms, medical history, and the clinical questions that need to be answered.<\/p>\n<p>Most people do not require both tests, although there are situations in which information from both examinations may be useful.<\/p>\n<p>The Marshall Speech and Hearing Center does not offer Modified Barium Swallow Studies as this test is completed at the local hospital with an xray machine.<\/p>\n<p>&nbsp;<\/p>\n<h2>What Happens After the Evaluation?<\/h2>\n<p>After your FEES evaluation, your speech-language pathologist will discuss the findings and recommendations with you.<\/p>\n<p>Recommendations may include:<\/p>\n<ul>\n<li>Continuing your current diet<\/li>\n<li>Modifying food texture or liquid consistency<\/li>\n<li>Changing positioning during meals<\/li>\n<li>Using individualized swallowing strategies<\/li>\n<li>Changing the size or pacing of bites and sips<\/li>\n<li>Beginning swallowing therapy<\/li>\n<li>Completing specific swallowing exercises<\/li>\n<li>Referral to another healthcare provider<\/li>\n<li>Additional medical evaluation<\/li>\n<li>Follow-up swallowing assessment<\/li>\n<\/ul>\n<p>Whenever possible, our goal is to identify recommendations that promote both <strong>swallowing safety and quality of life<\/strong><strong>.<\/strong><\/p>\n<p>&nbsp;<\/p>\n<h2>Swallowing Therapy<\/h2>\n<p>If treatment is recommended, swallowing therapy is individualized based on the specific physiologic impairments identified during the evaluation.<\/p>\n<p>Therapy may focus on:<\/p>\n<ul>\n<li>Improving tongue strength or coordination<\/li>\n<li>Improving tongue-base retraction<\/li>\n<li>Increasing pharyngeal strength<\/li>\n<li>Improving airway protection<\/li>\n<li>Improving coordination of breathing and swallowing<\/li>\n<li>Strengthening muscles involved in swallowing<\/li>\n<li>Practicing compensatory swallowing strategies<\/li>\n<li>Improving efficiency during meals<\/li>\n<\/ul>\n<p>Treatment recommendations are based on evaluation findings rather than using the same set of exercises for every patient.<\/p>\n<h2>FEES Services for Skilled Nursing Facilities<\/h2>\n<p>The Marshall University Speech and Hearing Center can also provide FEES services for individuals residing in participating skilled nursing facilities.<\/p>\n<p>Providing instrumental swallowing assessment within the facility may allow residents to receive a comprehensive evaluation without the burden of traveling to an outpatient hospital or clinic.<\/p>\n<p>Skilled nursing facilities interested in arranging FEES services may contact the Marshall University Speech and Hearing Center for availability and pricing information.<\/p>\n<p>&nbsp;<\/p>\n<h2>Insurance and Payment<\/h2>\n<p>When FEES is completed at the Marshall University Speech and Hearing Center, <strong>we bill insurance for the swallowing evaluation.<\/strong><\/p>\n<p>Coverage and individual out-of-pocket costs vary by insurance plan.
