Listed below are various forms from Human Resource Services and other
departments available for download or for completion on-screen. Users may access Human Resources Information System (HRIS) forms by clicking HERE.
Forms in AdobeŽ format are being converted to provide fields that can be filled in on-screen. To open a
form, scroll down to the selected form and click the word corresponding to the desired format:
PDF for AdobeŽ format;
WORD for Microsoft WordŽ format; and
ONLINE for on-line forms. If a user encounters any
problems with forms or requires any assistance in identifying and locating a needed form, please contact Human Resource Services at 304.696.6455 or by

NOTE ABOUT FORMATS:
Forms are typically available from this site in several
formats including Microsoft WordŽ, AdobeŽ
PDF, and
electronic on-line format. With WordŽ versions, the user may enter information into the
fields and blocks on the form from the desktop and print and/or save a copy. Opening AdobeŽ
PDF forms
requires at least the Adobe Acrobat ReaderŽ. If
a user does not
presently have the Acrobat ReaderŽ on
his/her computer, it may be downloaded
by clicking the icon below. The AdobeŽ
PDF forms are being
converted to fillable format. For all forms converted to fillable format, users may enter information directly into form fields from the
desktop and then print a copy to send through the mail.
Users are
cautioned, however, that if they fill out fields on the AdobeŽ
PDF form and
save the form, the fields may be blank when they re-open the form.
Therefore, it is advisable to fill out AdobeŽ
PDF forms in one desktop
session and then print out the form. If the user is running Acrobat
Reader but is unable to enter information at a block or field, the form
has not yet been converted to fillable format. Until then, print out the desired form and complete it with
typewriting or pen and ink.
