The student will need to have had the animal in their possession for at least three months prior to the request.
The animal will need to be fully house broken and all vaccinations will need to be current.
All requests require an ESA therapist evaluation. The student will need their treating counselor, therapist or psychologist complete the Exhibit A form.

Once you have the Exhibit A form completed and the animal’s updated vaccination records, you will need to complete this ESA Request form and attach both documents.

The animal will not be allowed on campus until the student has been approved through the ESA process.
