Persons interested in receiving services at the MUSHC may call the Clinic Administrative office at 304-696-3641. Prior to scheduling an evaluation, a case history form must be completed and returned to the Clinic office. Treatment recommendations are based on the results of the evaluation and scheduled accordingly.
