Insurance Refund Request Form

Please Read

The Office of International Student Services will assist you with beginning an insurance refund request. However, all refunds and refund decisions are handled directly by Insurance for Students and Aetna.

Submitting this request form does not guarantee that a refund will be processed.
Any questions regarding insurance coverage and refund policies should be directed to Insurance for Students directly at ifs@insuranceforstudents.com.

If you have graduated and plan to remain in the United States during your 60-day grace period or plan to apply for OPT:

Please be advised that you are allowed to continue your insurance coverage with IFS while you are in the United States. If you choose to continue coverage with IFS during your grace period or while on OPT, please contact them directly to discuss your options: ifs@insuranceforstudents.com

If you choose to continue coverage with IFS during your grace period or while on OPT, please do not submit this form.
