Campus Security Authority (CSA) Determination Form

This form is intended to assist in determining whether a campus official meets the criteria of a Campus Security Authority (CSA) under the Clery Act. Individuals who have significant responsibility for student and campus activities, or who are designated to receive reports of criminal offenses, may be considered CSAs and are therefore required to report certain crimes for Clery Act compliance. Please answer each question accurately. Your responses will help the institution ensure proper identification of CSAs and maintain compliance with federal reporting obligations. If you are identified as a CSA, you will receive additional information and training regarding your responsibilities.

1. Do you have responsibility for campus security or safety, even if you are not a member of a campus police or security department? (This includes individuals responsible for monitoring access to campus property or events.)(Required)
2. Are you a member of the campus police department or campus security department?(Required)
3. Do you have responsibility for reporting or responding to criminal incidents or emergencies on campus?(Required)
4. Are you an individual or office designated by the institution to whom students or employees should report criminal offenses?(Required)
5. Do you have significant responsibility for student and campus activities outside of the classroom? (Examples include student housing, student discipline, student organizations, or athletic programs.)(Required)
6. Do you advise or oversee student clubs, organizations, or Greek life?(Required)
7. Are you a coach, assistant coach, or athletic trainer for any university athletic team?(Required)
8. Do you supervise or coordinate student travel, including study abroad or off-campus trips?(Required)
9. Do you serve in a role such as a dean, director, or coordinator of student affairs, residence life, or student conduct?(Required)
10. Are you a faculty advisor to a student group or organization?(Required)

How to Interpret the Results: - If you answered Yes to any of the questions above, you are likely considered a Campus Security Authority (CSA) under the Clery Act. - If you answered No to all questions, you are likely not a CSA.

Important Update: Online Reporting Transition

Beginning Oct. 27, 2025:

The Title IX Office will move to an online-only reporting system. This change reflects a growing trend among Title IX offices nationwide and is designed to help us better serve our campus community.

Our goal is to ensure that Title IX reports are reviewed promptly and thoroughly. In-person and phone submissions, while often well-intentioned, can sometimes involve complex concerns that fall outside the scope of Title IX. These conversations, though important, can require significant time and follow-up, which may delay our ability to respond to Title IX matters with the urgency they deserve.

By centralizing reports through our secure online system, we can more efficiently triage concerns, prioritize Title IX cases, and maintain a consistent and equitable response process for all students, faculty, and staff.
