Formal Complaint Form (Title IX Athletics Equity)

This form is to be submitted to the Title IX Office to initiate a formal complaint of sex‑based discrimination in athletics (athletics equity) under Title IX. Instructions: If you believe you have experienced sex‑based discrimination in athletics, such as concerns about participation opportunities, athletics financial aid, or treatment and benefits, please complete this form, sign it, and submit it using the “Submit Formal Complaint Form” button below. Under Title IX and the Family Educational Rights and Privacy Act (FERPA), a parent or legal guardian may complete and sign this form on behalf of a minor. If you are not the Complainant and wish to report athletics equity concerns involving another individual, please contact the Title IX Coordinator directly. Only individuals who are currently participating in or attempting to participate in the University’s education programs or activities (e.g., students, employees, applicants) may initiate a formal complaint under Title IX. All athletics equity formal complaints will be processed in accordance with Title IX regulations. Retaliation for raising concerns or participating in a Title IX process is prohibited.

Please note: Title IX only applies to conduct that occurred within the United States and in the context of the University’s education program or activity.

TITLE IX JURISDICTION: Your complaint will be reviewed under Title IX’s athletics‑equity framework, including participation opportunities, athletics financial aid proportionality, and treatment and benefits in athletics, consistent with federal regulations. If the allegations do not fall within Title IX jurisdiction, the complaint may be referred to the appropriate University office for response under a different policy, and you will be notified of that referral. Because this form addresses sex‑based discrimination in athletics, and not Title IX sexual harassment, the mandatory dismissal and appeal provisions in the University’s Title IX Sexual Harassment Procedures do not apply to this process.
