Marshall University - Authorization to Release Educational Records for Student-Athletes

This form authorizes Marshall University to release a student-athlete’s educational records to another college or university in connection with transfer or recruitment processes. By completing this form, the student provides written consent under the Family Educational Rights and Privacy Act (FERPA) for Marshall University to release specified education records, including information related to academic standing, student conduct, and Title IX or other disciplinary matters, as requested by NCAA member institutions for due diligence purposes. This authorization is valid for one (1) calendar year from the date of submission and may be revoked in writing at any time, except to the extent that action has already been taken based on this consent.

Under FERPA, Marshall University may not disclose education records without a student’s written consent, except as permitted by law. By completing this form, you are providing consent for the release of the records described below.

NCAA Member Institution Requirements

The National Collegiate Athletic Association (NCAA) requires member institutions to exercise due diligence to identify and address Sexual Violence involving prospective student-athletes. As part of that due diligence, post-secondary institutions with NCAA athletic programs request information regarding Sexual Violence from each institution in which each prospective student-athlete has been enrolled. This information is crucial for ensuring the safety and well-being of all student-athletes.

Important Reminder: Online Reporting Strongly Preferred

Title IX Reporting - Online Submissions PREFERRED:

The Title IX Office prefers the submission of Title IX report through the online reporting system. It is preferred that all Title IX reports be submitted through our secure online form. This centralized process helps us review reports promptly, triage concerns efficiently, and ensure consistent and equitable responses for our campus community.

In‑person, email, and phone submissions are accepted on a limited basis as reporting methods for Title IX incidents. Those conversations may still occur for support purposes, but they do not constitute an official Title IX report.
