Now that you have registered for a Benefits Session, please review and complete the information below
The benefits packet provides an overview of all benefits offered to employees. Prior to the benefits enrollment session, we ask that you review and complete the documents in the New Employee Benefits Packet (for Marshall only employees) or the Dual Employee Benefit Packet (for Marshall Health and university employees). The forms you will complete are listed below:
- New Employee Drug-Free Workplace Receipt
- Voluntary Self-Disclosure Form
- Affordable Care Act Packet
- Authorization for Release of Information (if you wish to designate someone other than yourself to be able to access information on your benefits)
- Supplemental Information
- Health and Safety Receipt Form
During the in-person session, we will complete the online enrollment for your medical insurance, cafeteria plan benefits, and retirement. A couple of reminders for the benefit enrollment session:
- If you are opting for employee/children or family coverage, please bring a marriage certificate for a spouse and/or birth certificates for dependent children.
- Please have social security numbers handy for dependents and beneficiaries.
